I. Purpose |
This policy describes conditions for withdrawal from a course. It covers both official withdrawal and unofficial withdrawal. |
II. Accountability |
The student is responsible for adhering to the following policy in consultation with the Program Director and Associate Dean for Academic Affairs. The policy will be implemented by the SHP Registrar. |
III. Policy |
Official Withdrawal from a course Students who officially withdraw from a course prior to completion of ¾ of the course will receive a grade of "W" on their transcripts. Students must complete the Course Withdrawal form, obtain approval from the program director and file the form with the Registrar within this period. The official date of withdrawal is the date on which the completed form is filed in the Office of the Registrar, not the date of last attendance. Non-attendance or notification to the instructor/program director does not constitute an official withdrawal. Upon approval of course withdrawal, the course instructor will be notified of the withdrawal by the Registrar. Unofficial Withdrawal from a course A student will be considered to have withdrawn unofficially from a course or courses if he/she never attended class and did not officially drop the course or is absent for three consecutive web modules, didactic/clinical days or more without notification to the appropriate course instructor or program director. Unofficial withdrawal from a course will result in a grade of "WF" if the student withdraws prior to completion of 3/4 of the course. If a student unofficially withdraws after 3/4 of the course has been completed, a performance grade will be issued. Administrative Withdrawal from a course Students withdrawn from a course by the School for reasons including but not limited to, financial reasons (student has not paid tuition and fees due) or health reasons (student has not completed required immunizations.) will receive a grade of "W". Tuition and Fees A student who withdraws either officially or unoffically after the tuition refund period will be assessed full tuition and fees. |
