I. Purpose |
This policy insures that students are notified regarding the University's Policies on Immunization and Immune Status. |
II. Accountability |
The Program Director and Associate Dean for Academic Affairs are responsible for this policy. It is the responsibility of individual students to follow this policy. |
III. Policy |
All SHP students are required to submit documented proof of immunization; the extent of immunization is based on the extent and type of interaction with patients, bodily fluids and other students. The policy and forms are available on the University website. If immunization conflicts with the student's religious beliefs or is medically contraindicated, the student should submit a statement of the same to the University. However, the University imposes strict guidelines in regard to such conflicts. |
